我自願參加正道福音神學院（LES）『教會蓬勃發展中心 (TCC)』2026年10月19日~20日的《靈澄之旅》靈修會，明白活動可能因意外、自然因素或其他無法預見的情況導致受傷、疾病甚至死亡。儘管我目前健康良好，仍願意承擔所有相關風險。我代表本人及代理人，放棄對正道神學院及其負責人、同工、志工的一切法律追訴權，並承諾不因任何損害或損失而提出索賠，無論是否因對方疏忽所致。 As a participant on a Logos Evangelical Seminary (LES) 『Thriving Congregation Center (TCC)』，Spiritual Formation Retreat》 on October 19~20, 2026, I realize that inherent dangers exist. While in good health and able to fully participate in such activities, I realize my participation may result in illness or injury due to accidents, the forces of nature or other unforeseen causes. Such illness or injury may include disease, strains, sprains, fractures, dislocations, paralysis and/or death. By participating, I hereby and knowingly assume all risk resulting from these activities. On behalf of myself, my family or other personal representatives I hereby agree to release, hold harmless and indemnify the LES, its agents, officers and activity leaders from any and all claims and law suits for bodily injury, property damage, wrongful death, loss of services etc. which may result from my participation in the above mentioned activities, regardless of whether or not these claims or suits arise from negligent acts, omissions by the LES organizers, leaders or facilitators of the activity, employees or volunteers, another participant, any other person involved or from any other cause.
